Mukhya Mantri Loksevak Arogya Yojana (MMLSAY)
Mukhya Mantri Lok Sevak Arogya Yojana is a medical reimbursement scheme of the State Government which aims at providing medical reimbursement to Government employees including Government pensioners and their dependents at CGHS rates.Registration and reimbursement process can be done either by self or through assistance of the District Kiosks located in O ices of the District comissioners. Certain information like Aadhaar Number, Contact Number and family details will have to be provided at the time of registration. For submitting claims, bank details and documents related to treatment in the concerned hospital are to be uploaded.
Documents required in order to register with MMLSAY?
Details of Aadhaar, Contact number, mail id, residential address, designation, PAN Number, department ( last designation and department in case of pensioners), dependent details, Bank account details of self and nominee, PPO details and PPO number( in case of pensioners) are to be uploaded in case of registration in the portal.
